While submitting a Report you may notice that Step 1 may be Orange and have an Exclamation mark, meaning it needs attention. Please follow the below steps for editing Step 1 of the Report.
While in the report, Click Edit on Step 1: Review Facility Information. Its on the right side of the blue bar:
There are three Tabs in Step 1, each Tab will need to have the information verified:
Location and Business Activity at this Site tab:
Verify the following Fields have the correct Facility information:
- Facility Name and Department/Division
- Facility Address
- Click the Get Lat/Long button to validate Latitude/Longitude (Move the red marker on the map so that it is directly over your facility, then click the Pick this Location button.)
- Select the correct LEPC from the drop down menu if your county has one
- Select the Fire Department that would respond to an incident at your facility
- Tax ID number
- Answer the State/Local Government question
- Answer the Agricultural Facility question (Click "What is This?" for field explanation.)
- Click the "Search NAICS" button to search the facility's industry to populate the NAICS 1 and the NAICS 1 Description fields. Do this for the NAICS 2 field if there is a secondary NAICS code for the business activity. Fill out the Description of Business Activity field
- Mark Occupied/Unoccupied
- Enter the Maximum Number of Occupants at one time
- The Dun & Brad number is a required field (If your company doesn't have a Dun & Brad number, enter N/A.)
- Answer Yes/No to the remaining questions (If your facility has a Written Emergency Plan, provide the plan location.)
- Save.
- If the Save does not move to the Owner/Operator tab and you receive a Pop Up message concerning the Address, Check the box above the Facility Name ‘Check here if Address is not a Standard Address’,
Owner/Operator tab:
- Check the Direct Site Communication Phone Number.
- Review the Parent Company Address. (Remove incorrect information and fill in any fields with asterisks *.)
- Confirm the Owner/Operator Information is accurate and fill in any missing fields
- Answer the Multiple Addresses question. (If your facility has contiguous addresses, add them.)
- Save.
Mailing/Billing Address tab:
- Confirm the Mailing Address listed is correct (If absent fill in fields with asterisks *)
- Enter the Billing Address. (If absent fill in fields with asterisks *)
- Save.
