Log into CHS Manager with your Username and Password.
- At the top of the screen hover over the "Facilities" tab and select "List Facilities".
- Locate the Facility you wish to report on (in blue) and click on it.
- Click onto the “orange outlined box” to the right of the screen to add a report.
- You can choose to submit an Annual report (drop downs have year choice), Update or a Revision. Select the appropriate report and then go to Proceed.
