To locate the Fee Summary for a submitted report, follow the steps below:
1. Log in to CHS Manager. On the Home page, hover the mouse over Facilities and click List Facilities.
2. In the Facility Name column, click on the Facility Name hyperlink
3. On the List Submissions page, locate the latest Annual/Revision Report then click the View icon on the same row.
4. The Fee Summary details are located at the bottom of the report. Clicking on the Fee Summary No. will open the Fee Summary invoice as a .PDF document.
