A substantive change is submitted through an update report, whether or not you have submitted your annual report.
- Log into CHS Manager and choose your facility from the Active Facilities list
- Click the "Add Report Button"
- Select "Update". You can only select the current year. See the article "What's the difference between annual, update, and revision reports?"
- Complete the form with your updated information.
- Once you have completed steps 1 through 6 listed at the top, submit your report (Step 7).
